The Farmer's Exchange is looking for nominees for its annual Exceptional 4-H'er Award, which seeks to recognize those in 4-H who have overcome obstacles and adversity to excel in their 4-H endeavors.
To nominate a 4-H'er, please complete, in detail, both pages of the form below, print and fax to 574-831-2131, Attention:
Courtney Schafer; or they can be e-mailed to courtney@farmers-exchange.net. Those who prefer to mail in their nominations can send them to Courtney Schafer, PO Box 45, New Paris, IN 46553.
Feel free to forward or give this form to others, as anyone can nominate deserving 4-H'ers for this award.
All nominations must be received by The Farmer's Exchange by May 26, 2017.
